From 1st July 2025, changes from the Department of Health mean that access to Chronic Disease Management plans may depend on your MyMedicare registration.
What is MyMedicare?
MyMedicare is a voluntary program that lets you nominate CHC Medical as your regular practice. This strengthens your relationship with our team and helps us deliver more personalised, continuous care.
Note: You still need a Medicare card to access health services — MyMedicare simply links you to a preferred practice.

Who Can Register?
To be eligible, you must:

Hold a valid Medicare or DVA card

Have visited CHC Medical at least twice in the past 24 months
